These three treatments overlap, and the difference between them is mostly a question of how much of the tooth needs to change and how much force it has to survive.
Composite resin is added directly onto the tooth in a single appointment. In most cases little or no healthy tooth is removed, which makes it the least invasive and the least expensive of the three, and – importantly – a reversible starting point. It suits chips, small gaps, worn edges, uneven lengths and isolated discoloured patches. It is shaped by hand in your mouth, so the result depends heavily on the operator’s eye rather than on a laboratory.
Porcelain veneers are thin shells made in a laboratory and bonded to the front of the tooth. They hold their colour permanently, resist staining, and last significantly longer than composite. The trade-off is that a layer of enamel is usually removed, they take two or three visits, and they cost more. They are the right answer when several front teeth need their colour, shape and surface changed together, or when deep internal staining will show through composite.
A crown covers the whole tooth. It is a restorative decision rather than a cosmetic one: a tooth that has been root-treated, has a large old filling, or has cracked needs the wrap-around strength that neither bonding nor a veneer can provide.
Where a case genuinely sits on the boundary, we usually suggest trying bonding first. If it lasts, you have saved a great deal of money and kept your enamel. If it does not, nothing has been lost and veneers remain available.
Bonding performs extremely well on the front teeth, where the loads are light and the demand is aesthetic. Closing a small midline gap, rebuilding a chipped corner of an incisor, lengthening lower front teeth that have worn flat, and evening up two teeth of slightly different lengths are all jobs it does beautifully in one sitting.
It is less suited to a few situations, and we would rather say so up front. Composite is not the material of choice for very heavy biters or people who grind, because the material is softer than porcelain and will chip; if we do bond in those cases, we will insist on a night guard. It cannot mask very dark underlying stain, because the resin is slightly translucent and the darkness reads through. Large gaps – more than a few millimetres – are usually better closed by moving the teeth with aligners rather than by adding wide shelves of composite that trap plaque. And composite picks up stain at its margins over the years, so if you drink several coffees a day or smoke, expect to have it polished and refreshed more often.
Bonding is the treatment patients most often describe afterwards as “that was it?”. There is usually no injection, because we are adding to the tooth rather than cutting into it. There is no drilling in most cases – the surface is roughened and conditioned so the resin grips, which feels like nothing at all.
We agree the shade with you in daylight before starting, then build the composite up in thin layers, curing each with a blue light. Shaping and polishing take as long as the building does, and this is where a natural result is won or lost: the tiny ridges, the slight translucency at the edge, the way light catches the surface. Near the end we sit you up and hand you a mirror while the material can still be adjusted, because how a tooth looks when you are upright and smiling is not how it looks lying flat under a bright lamp.
One tooth typically takes 30 to 60 minutes. Four to six front teeth – an “edge bonding” or “composite makeover” case – is usually a single longer appointment of two to three hours. You eat and drink normally straight afterwards; there is no setting period to wait out.
Composite is strong but not invincible, and almost every failure we see comes from one of three habits. Biting nails, pens or ice puts a point load right on the thinnest part of the restoration. Opening packaging with your front teeth does the same. And grinding at night applies far more force than chewing ever does, which is why a guard is worth having if you grind.
For colour, the first 48 hours matter most, as freshly cured composite is slightly more absorbent: go easy on red wine, curry, beetroot, dark berries and strong tea and coffee. After that, ordinary care is enough – brush twice daily, floss around the bonded areas, and keep your hygiene appointments, where we polish the composite back up as a matter of routine. Avoid whitening toothpastes marketed as “abrasive” or containing charcoal; they dull the polish and make composite stain faster, not slower.
One point worth planning for: composite does not lighten when you whiten. If you are considering whitening, do it first and let the shade settle for a couple of weeks, then bond to the new colour. Doing it the other way round means the bonding will look darker than the teeth around it.
Realistically, expect five to eight years from front-tooth bonding, with some cases running well past ten and heavy grinders getting less. Failure is rarely dramatic. Usually a margin starts to stain and outline the edge of the restoration, or a small piece of the edge chips off.
Both are straightforward to deal with, and this is one of bonding’s genuine advantages: it is repairable in a way that porcelain is not. We can add to it, re-polish it, or replace the section without starting again on the whole tooth, usually in a single short visit. If a chip happens, keep the tooth clean and book in – there is no urgency and no risk to the tooth itself.
Bonding is priced per tooth, and the fee depends on how much of the tooth is being rebuilt: a small chip on one corner is a much smaller job than lengthening and reshaping an entire incisor. Coverage depends on why the bonding is being done, and insurers draw the line at necessity rather than appearance. Repairing a genuinely broken or decayed tooth is a restorative procedure and is commonly covered by private plans and by the Canadian Dental Care Plan. Reshaping teeth that are sound but not the shape you would like is cosmetic and usually is not. Many cases are a mixture, and we itemise the estimate so it is clear which part sits where.
